It should be noted that Dziner Holidays is only responsible for the submission of the passports. The delivery time of the passports / acceptance or non acceptance of a form is at the sole discretion of the passport office.
It is the applicant’s liability to fulfill all the requirements for the passports as asked by the passport office failing which we don’t guarantee the issue of the passport.
The delivery time of the passports is also not a responsibility of Dziner Holidays.
GENERAL DOCUMENTATION FOR A PASSPORT

ADULTS MAKING PASSPORT FOR THE 1ST TIME

• 10 Passport Size Photographs (35 mm X 35mm)
• 2 Address proofs – 1 for current year 1 for previous year like Ration Card / Gas/ Electricity/Telephone bill /Wera Receipt/Bank pass book bearing name and address/Society chief letter for address proof.
• Birth Certificate / School Leaving Certificate bearing place and date of birth. Applicable Affidavits: - S.L.C Duplicate Affidavit, No Birth Place Affidavit, Duplicate S.L.C Affidavit.
• Marriage Certificate if married. Applicable Affidavits :- Marriage Affidavit
• No objection Certificate if in a Government Job.
• Authority Letter and Declaration Form

ADULTS HAVING AN OLD /EXPIRED PASSPORT
• Proof of Birth place and Birth Date not required.

ADDITIONAL REQUIREMENTS FOR MINORS
• Minor Affidavit
• School Bonafide Certificate
• Parents Passports if Applicable.

All documents in original and three photocopies with signature in all. Signature should be in the center and not at the bottom. In case of Minors either parent’s signature is required.
